Monthly membership at Clarity Direct Care is $65 for ages 0 to 44, $85 for ages 45 to 64 and $105 for ages 65 and over, and $35 for children 0 to 18 with an adult member.
There is a one-time, non-refundable enrollment fee of $85 per person or $175 per family.
Membership includes 30 to 60 minute appointments, chronic disease management, urgent care visits, mental health support, pediatric and newborn care, and in-office joint injections and EKGs at no extra cost.
