For ages 25 to 34, the retainer is $75 a month or $900 a year.
For ages 35 to 44, the retainer is $100 a month or $1,200 a year.
For ages 45 to 54, the retainer is $125 a month or $1,500 a year.
For ages 55 to 64, the retainer is $150 a month or $1,800 a year.
For ages 65 and over, the retainer is $175 a month or $2,100 a year.
Each age group includes the annual physical exam and all necessary office visits.
The fee can be paid monthly, quarterly or yearly by bank draft or debit or credit card. A 4% administrative fee is added to credit card payments, and paying yearly before the due date earns an extra month free.
