Homebound patients of any age pay $150 per month (call first, subject to availability).
The family discount is $225 per month for 2 parents and up to 3 children under 18, then $20 per child per month for additional children.
Paying for the year earns a 5% discount, and full-time college students pay $300 per semester.
A one-time registration fee of $75 per member or $150 per household applies.
