Homebound patients (home visits) are $350 per month, and the family maximum (up to two adults and dependent children) is $450 per month. There is a one-time enrollment fee of $75 per individual or $150 per family, a 3-month minimum membership, and a 10% discount for veterans, first responders, and active-duty service members.
