About Dr. Feldman
Dr. Feldman earned her medical degree at Rosalind Franklin University of Medicine and Science. She completed residency at the MacNeal Hospital Family Medicine Residency in Berwyn, Illinois. During residency, she spent time with Dr. Andrew Weil's program in Integrative Medicine in Tucson, Arizona.
She is board certified in family medicine and specializes in integrative and functional medicine. After residency in 2008, she opened an insurance-based integrative primary care clinic in La Grange, Illinois, called Integrative Family Health Associates. In 2015, she created Vitality Wellness Partners, a functional wellness practice outside of the insurance model.
She opened Vitality Family Health in 2018, which chose a Direct Primary Care membership model. That same year, she began coaching other healthcare practitioners across the country as a Hybrid Practice Specialist.
Membership and fees
At Vitality Family Health, the Adult Maintenance membership is for the generally healthy adult not on prescription medications.
It costs $150 per month and includes 4 visits per year, and each additional visit is a flat fee of $75.
The Adult Restorative membership is for the adult on prescription medications and costs $250 per month with an unlimited number of visits.
The Adult Hormone Optimization membership costs $350 per month and includes the Adult Restorative membership with unlimited visits, plus 2 additional lifestyle coaching visits with Dr. Feldman each year.
It also includes comprehensive blood work panels and hormone testing, including pap smears, as indicated.
The Pediatric Add-On membership for ages 0 to 24 requires at least one parent or guardian to be a member, and the first child is $75 per month and each additional child is $50 per month, with unlimited visits.
