About Dr. Daisy

Dr. Daisy is board-certified in Family Medicine and Geriatrics. She provides relationship-based primary care for children, adults and families. Her geriatrics training prepares her to meet the health needs of older adults within their established medical home.

She served as president of the Illinois Academy of Family Physicians in 2023-2024. As of 2023, she was on the Illinois Academy of Family Physicians Government Relations committee and co-chair of its Women in Leadership Member Interest Group. She remains active in both the Illinois and American Academies of Family Physicians.

Dr. Daisy wants patients to feel heard, supported and empowered. She believes every patient deserves care that is compassionate and fits their goals and needs.

Bridge Family Health offers a direct primary care membership that does not cover specialist care, hospital, imaging or prescriptions.

Monthly membership is $85 for one adult, $140 for one adult and one child, $160 for two adults in the same household and $200 for two adults and one child.

Each additional child on the family plan is $35 a month, and the one-time sign-up fee is $100 for an individual or $200 for a household.
