Dr. Foulk received his undergraduate degree from Brigham Young University and graduated from the University of California, San Diego School of Medicine in 1990. He completed his obstetrics and gynecology residency at the University of California, Irvine in 1994, and a fellowship at the University of California, San Francisco in 1998.
