Membership and fees
One-time enrollment fee of $99
Monthly membership fees: $59 for an adult, $54 for a spouse, and $25 for a dependent child in the same household (with at least one paid adult membership)
$20 per-visit fee in addition to the monthly or annual membership fee
Annual prepay membership options: $650 for an adult, $600 for a spouse, and $300 for a dependent child (with at least one paid adult membership)
Non-members choosing a la carte visits are charged $129 for acute care problems and $225 for chronic care management
HSA funds cannot be used to pay the membership fee itself but can be used for office visits, physical exams, labs, x-rays, and EKGs
Membership fees can be paid monthly by automatic card or EFT enrollment or paid annually for savings
Requires 30 days' written notice to cancel a membership
