Monthly membership is $60 for ages 0 to 20, $100 for ages 21 to 64 and $125 for ages 65 and older, and prices are guaranteed for the first full year of membership, based on start date.

Membership includes unlimited in-office and virtual visits, and labs and imaging can be paid at the practice's negotiated rates.
