About Lynsey Mahlum

Lynsey Mahlum is a board-certified nurse practitioner and the founder of Rooted Health Primary Care. She holds the ANP-C and CHFN credentials. She completed her nursing training at Humboldt State University (BSN) and her Adult Nurse Practitioner master's at the University of Colorado Health Sciences Center.

She started her career in cardiac thoracic ICU and internal medicine nursing. She has built a background in primary care, chronic disease management and functional medicine. She has held leadership roles, such as managing one of the largest outpatient clinics in Colorado.

She has also cared for geriatric patients in post-acute settings. Her practice offers primary care along with wellness services such as hormone optimization and weight management. The practice emphasizes longer appointments and evidence-based care that focuses on prevention.
