New patients at Perennial Wellness start with a $695 Baseline Assessment, and the full amount counts toward membership if they continue into ongoing care.
The Advanced Longevity membership is $500 a month, billed monthly, with four scheduled physician visits a year and a DEXA scan, VO2 max and resting metabolic rate included, while labs and outside testing are billed separately.
The Perennial Concierge membership is $2,100 a month and includes unrestricted year-round access and urgent visits.
