Incompetent or malicious?
I'm not sure is she is incompetent or malicious. Cannot read an X-ray or be trusted with patient confidentiality. She deliberately notated the cause of visit incorrectly due to her ideological differences with a patient, causing years of insurance problems and harming the patient in the process. Choose a different provider.
